Healthy Planet Link

Healthy Planet Link (HPL) is Epic’s Population Health Solution, used to provide care management tools to community providers outside of AdventHealth and who do not use the same EHR platform. Outside providers can include referring physicians, referred‐to physicians, contracted physicians, and physician and support staff delegates of organizations who might need to review clinical and administrative information for their shared patients.
